Licensed clinicians can come to you

Aetna Medicare has partnered with specific vendors to offer healthy home visits on its behalf  — in the comfort of your home. This program doesn’t replace your relationship with your primary care physician (PCP). Instead, it helps Aetna work with your PCP to manage your care and direct you to the health programs and services you need.

  • The benefit is offered at no extra cost to you

  • Participation in this program does not impact your coverage

  • Our partner Signify Health will contact you. First, they’ll send you an introductory letter followed up with a phone call to schedule your visit

  • The licensed clinical professional will visit you in the comfort of your home at the scheduled time 

  • After the visit, we’ll mail you a summary of your visit. We’ll also send a copy of the summary to your PCP

Aetna remains dedicated to your health and well-being. our health care professionals are following CDC Covid preventive measures, such as wearing masks and gloves at each visit and maintaining a safe distance. Virtual healthy home visits with audio and video options may also be available.

Our healthy home visit is just one of the many ways we’re committed to being there for you every step of the way.

 

During your optional healthy home visit, you can ask a nurse practitioner or doctor any health question on your mind.

 

It’s also a way for us to discover things that don’t always show up during a standard doctor visit.

 

For example, we perform a medication review so we can make sure you clearly understand what you’re taking and can advise you of any drug interactions. The nurse can also let you know if there is a lower cost, generic equivalent available for a medication you may be taking.

 

Your doctor will get a summary of the visit. and if the nurse has any immediate concerns, they will follow up with your doctor.

 

During the healthy home visit, we’ll provide education about nutrition and how eating well can affect your health, especially with health conditions like diabetes.

 

And if we see someone facing physical or financial challenges with gaining access to healthy food, we can connect them to helpful local resources.

 

We want to help keep you safe and healthy in your home. so, we search for tripping hazards and provide education on how to prevent falls with every visit.

 

And because health is more than just physical wellness, we also offer emotional support if needed. with loneliness affecting many retirees, this is an important part of the program.

 

If you feel lonely or depressed, we can help connect you with resources and programs that may help you feel better.

 

Our healthy home visit is one of the first steps you can take in the journey to supporting your whole health.

 

From actions like ensuring you’re up to date on screenings, to introducing you to wellness programs, and even checking that there’s enough light in the hallway.

  • Talk about getting the health resources you need 

  • Discuss how to set up a safe, healthy home

 Because your health is always our top priority, we take many steps to make sure you feel safe at home. Our licensed clinicians follow all Centers for Disease Control and Prevention (CDC) guidelines and safety measures including, but not limited to, social distancing with anyone else in the household and wearing face masks and gloves.
