Vision reimbursement
You are eligible to receive up to $175 reimbursement every 24 months on lenses, frames or contact lenses. You choose where to buy your eyewear and submit to Aetna for reimbursement.
To submit a claim, mail us your completed form to request reimbursement. For more information, contact Member Services at the number on the back of your Aetna ID card.
