Aetna Healthy Rewards

Your plan includes Aetna Healthy Rewards. Here’s how it works:

• Receive program information by mail or email.

• Log in below or go to CVS.com/Aetna to register or log in and access your rewards. Or you can call 1-844-428-8147 (TTY: 711), 8 AM–8 PM local time, Monday–Friday except on federal holidays.

• Complete your recommended health care activities listed below:

   

  • $50 - Schedule and complete an annual Healthy Home Visit.
    
    
  • $40 - Complete an annual wellness visit or physical exam with your primary care physician (PCP).
    
    
  • $50 - Get a preventative screening for depression, breast cancer or colorectal cancer.
    
    
  • $10 - Get an eligible vaccine: annual flu shot, COVID-19 vaccine or booster, HEP-B vaccine or pneumococcal vaccine.
    
    
  • $50 - Get an A1C test, annual eye exam and confirm you’re following your prescribed diabetes care plan.

• Tell us you’ve completed the activity either online or by phone. We’ll just need a few details, such as the date of the visit and the clinic name. 

• Redeem your first reward and you’ll receive a reloadable Extra Benefits card in the mail within 10-14 business days. Your rewards funds will be loaded to the Healthy Rewards Wallet on the card. You can use it at many participating locations for:

  •  Healthy foods
  •  Transportation
  •  Utilities
  •  Personal care products
  • Over-the-counter (OTC) health and wellness products

• You’ll also receive a member guidebook with your Extra Benefits Card after redeeming your first reward. The below includes a sample of the guide.

• Do not throw away your card once you’ve used the funds in your Healthy Rewards Wallet. You will keep using the same card next year.
