Our Healthy Aging Support Program will guide you with practical strategies to help you take care of your health and get the most from your benefits.

What to Expect

What to Expect

We have enrolled you in either Group Coaching or Individual Coaching, based on your needs. An Aetna® nurse will call you to assess your health needs so your health coach can better serve you. Our nurse will tell you which program type you’re enrolled in.

Give another person permission to help manage your care

We take protecting your medical and personal information seriously. Without your consent, we’re limited in how we can work with your caregivers or family in case you can’t function normally. Or if you’re admitted to a:

  • Hospital
  • Rehabilitation facility
  • Skilled nursing facility

Working with your caregivers or family is a great help during these critical times.

We have a form called the Authorization for Release of Protected Health Information (PHI) you can complete each year. It will let us work with anyone you want to take part in important decisions such as discharge planning or other concerns.
