{VO:} Hi, I’m Rebecca, Barbara’s care manager. As part of her support team, I’ve gotten to know her well.

{VO:} To begin our relationship, Barbara’s care coordinator gives her a call to learn about her health, living and social needs.

 

{VO:} Then Barbara’s personal care team — which includes a care coordinator, a social worker and me, her care manager — review this information. We work together to create her personalized care plan and share in her path to better health.

 

{VO:} Barbara’s care team doesn’t end there. She also has a doctor, nurse, pharmacist, and mental health expert, all joining together to coordinate her care.

 

{VO:} After speaking with Barbara, we learned she has diabetes. But she hasn’t been able to see her doctor for a while because she doesn’t have transportation.

 

{VO:} With that critical information, I reached out to her doctor. I was able to immediately schedule an appointment for a checkup — to review her current health status, diabetes medications and to make sure her tests are up to date. I also set up transportation to take Barbara to and from her appointment.

 

{VO:} The initial phone call was just the start of our relationship. I frequently check in with her to see how she’s doing. This includes making sure her diabetes medicines are filled, and that she’s on track with her testing needs. I also help connect her with resources in her community so she can access healthy foods, and help her order supplies or medications and have them delivered to her home. As I’ve gotten to know Barbara better, I’ve learned that she’s an ace bridge player. So I’ve connected her with a nearby community center that hosts a weekly bridge game.

 

{VO:} With her care plan in place, Barbara’s diabetes is under control and she’s feeling a lot better. And she’s making new friends, too. Aetna is committed to helping members like Barbara every day.
