Welcome to the Aetna® Dual Eligible Special Needs Plan, or D-SNP!
Our No. 1 goal: making it easy to get the care you need. And we start right away with a welcome call from a member of our team. They’ll confirm your plan and help answer your questions.
You will receive a welcome kit in the mail. Inside you’ll find your Quickstart Guide. It includes important benefit information and tips to help you get the most from your plan.
You will also receive your Aetna® member ID card by mail. It will include your primary care provider and the Member Services number. Keep it safe and always bring it to pharmacies and doctor visits.
Once your plan begins, you can take advantage of the medical and prescription drug coverage, plus coverage for dental, vision and hearing.
All Aetna® D-SNP plans also come with a quarterly over-the-counter (OTC) allowance. This allowance can be used to buy approved wellness items, such as pain relievers, cold and flu medicines, first aid items and more.
Check your welcome kit for your OTC catalog. It shows a wide selection of approved health and wellness products you can order using your quarterly OTC allowance.
Most plans come with a transportation benefit and a healthy foods allowance. The transportation benefit includes rides to and from approved locations like the doctor’s office, pharmacy and senior centers.
If your plan includes a healthy foods allowance, you will receive a Healthy Foods Card in the mail, along with information on how to use it. You can expect this soon after your plan starts.
The Healthy Foods Card is a debit card with a monthly allowance. You can use your card or download the free Healthy Benefits Plus app to buy approved foods like fruits, veggies, dairy products, meats and seafood, and pantry staples.
As an Aetna D-SNP member, you also have a personal care team. They are there to make getting the care you need easier. A member of your care team will give you a call after your plan starts to get to know you and help you access the care and programs that are right for you.
Your care team can help you schedule doctor visits. They can connect you with local programs that can help you find healthy foods, safe housing and more. You can reach out to your care team with any questions about your health.
Have a health question after hours? No problem. You can call the 24/7 Nurse Line for answers anytime, including nights and weekends.

You can get the health and wellness products you need at no extra cost. Here’s how it works.
Your OTC allowance becomes available to you as soon as your plan begins, and it renews every quarter. To start shopping, look through your OTC catalog that comes in your Aetna D-SNP welcome kit.

Ready to order? Grab your Aetna® member ID card. You’ll need it to use your allowance.
There are three ways to order:
If you ordered online or by phone, we’ll mail your items directly to your home. You can place up to three orders per quarter. It’s easy to check your balance in your online account. Just be sure to use your allowance before it expires — it doesn’t roll over to the next quarter.

Eating the right foods can help you stay healthy.
And healthy eating can be easier with the Aetna® Healthy Foods Card.
The Aetna® Healthy Foods benefit card has a monthly allowance that can be used to buy approved food items at select stores.
The card will arrive in the mail shortly after your plan starts. It will include instructions on how to activate the card and how to use it to purchase the healthy foods you need.
You can activate your card in three ways:
1. By phone. Simply call the number on your card.
2. Online. Go to HealthyBenefitsPlus.com/AetnaMedicare
3. By mobile app. Download the free Healthy Benefits Plus app.
Once your card is activated, go grocery shopping! Approved items include fruits and veggies, meat and seafood, healthy grains like bread and pasta, pantry staples like flour and spices and more. To find out if an item is approved, use the in-store scanner in the Healthy Benefits Plus app.
Ready to check out? Your benefit card works just like a coupon.
The clerk will ring up your order and then scan your card or the barcode in your Healthy Benefits Plus app.
You can use your card along with regular coupons and other food assistance programs.
If your allowance covers your full purchase, you’re good to go. If any of your items aren’t approved (or if you’ve used up your allowance), you pay the remainder.
Short on time? You can use your allowance to have fresh and healthy meals delivered to your home from Mom’s Meals. To learn more about your meal choices or to order, call 1-877-347-3438 (TTY: 711). Or go to Walmart.com to order approved foods for home delivery or curbside pickup. You may need to use another form of payment for shipping and delivery fees, which are not covered by your allowance.
Check your balance before you shop to see how much you have to spend. You can check your balance online, by phone or in the Healthy Benefits Plus app.
Use your card as often as you’d like until your allowance is used up. An allowance does not roll over, so be sure to use it before it expires at the end of the month.
If you didn’t get your card or if you need a replacement, contact the Healthy Foods Card benefit vendor at 1-833-451-4662 (TTY 711). Your Aetna® Member Services team can answer any questions about the card. Call them at 1-866-409-1221 (TTY 711).
