A member of our team will call you to:
  • Confirm your plan
  • Review your primary care physician (PCP)
  • Help answer your questions

 

Welcome kit

You’ll get a welcome kit in the mail. It will include:
  • Quick Start Guide
  • Over-the-counter (OTC) catalog
  • Caregiver form

 

Member ID card

Your Aetna member ID card will arrive by mail. It will have:
  • Your primary care physician (PCP)
  • Important phone numbers

Always bring your ID card (and your Medicaid ID if you have one) to pharmacies and doctor visits.

You can get your plan information anytime, anywhere. Just sign up for your personal Aetna member website. Then you can log in to:

 

  • View your ID card
  • Choose a primary care physician (PCP)
  • See what your plan covers
  • View past doctor visits
  • See your recent prescriptions and more

Meet your D-SNP care team

 

As a D-SNP member you have the support of a personal care team. Your team will help you get the heath care you need, when you need it.

Your personal care team includes:

  • Care coordinator
  • Nurse care manager
  • Social worker
  • Member advocate
  • Health survey specialist

 

And they can:

  • Learn your individual needs
  • Develop your care plan with your doctor
  • Coordinate visits to doctors, dentists and other health care providers
  • Help you understand your medications
  • Connect you with programs to help with safe housing, healthy foods, utility bills and more
  • Help you access your state Medicaid benefits

That’s why we are offering a $50 reward when you complete an easy health survey. Call 1-866-409-1221 (TTY: 711) and press option 2 to reach a Health Survey Specialist, Monday to Friday 8:30 AM to 5:00 PM, local time.

Taking care of your health and well-being is a lifelong journey. As a member of an Aetna Dual Eligible Special Needs Plan, or D-SNP, you don’t have to go through it alone. You have an entire Aetna care team on your side every step of the way.


Your care team may include a nurse care manager, care coordinator, social worker, member advocate and health survey specialist. They are there to help you manage your health and get the most out of your benefits.


Soon after your plan begins, a member of your care team will give you a call to complete your health survey. This will help them get to know you and your health and wellness needs. And once it’s complete, you’ll receive a $50 gift card in the mail. Since your needs can change over time, the survey should be updated each year — your care team will reach out each year when it’s time.


Are you managing one or more health conditions? Struggling with certain symptoms? Confused about your medications? Sharing your questions and concerns with your care team can help them develop a care plan that best fits your needs.


That might include questions and concerns about seeing more than one health care provider. But don’t worry. Your care team may be able to help coordinate and schedule visits to your doctors, the dentist and other providers you need to see.


Having trouble getting to an appointment? Your care team may be able to help with that too. Most plans include transportation to and from approved locations. These include doctor’s offices and hospitals, pharmacies, senior centers and more.


Many D-SNP members have questions about their medications, like: How often should I take it? What are the side effects? What happens if I miss a dose? Understanding your medications is important for staying well. And your care team can help provide answers.


Beyond health care services, you also need a safe place to live to stay well. If that’s missing from your life, your care team can provide assistance. They can connect you with local and state programs that can help you with safe housing, your utility bills and more.


Sometimes, understanding your Medicaid benefits can be hard. If you have questions, your care team is there to answer them. They can help you access your state Medicaid benefits and make sure you get the benefits and services you need.

Get the most from your benefits

 

Your Aetna coverage includes medical services and prescription drugs. Plus extra benefits like:

 

$0 copay for covered prescription drugs at in-network pharmacies

Annual benefit to use toward in-network dental services such as X-rays, cleanings, fillings, crowns, extractions, root canals, implants and more

Annual eye exam with an in-network provider and an allowance for non-Medicare covered prescription eyewear, including glasses, lenses, frames or contacts

A quarterly allowance that can be used toward healthy foods

Annual hearing exam plus an allowance for hearing aids per ear, plus a $0 routine hearing exam with an in-network provider

A quarterly allowance that can be used toward over-the-counter (OTC) health and wellness products**
