Meet your D-SNP care team

 

When you’re a D-SNP member, you’ve got the support to get the benefits and services you need.

Your personal care team includes:

  • Care coordinator
  • Nurse care manager
  • Social worker
  • Member advocate

 

And they can:

  • Learn your individual needs
  • Develop a care plan with your doctor
  • Coordinate visits to doctors, dentists and other health care providers
  • Set up transportation to and from appointments*
  • Help you understand your medications
  • Connect you with local and state programs to help with safe housing, healthy foods, utility bills and more
  • Help you access your state Medicaid benefits

Your care team will call you to get to know you and complete a short health survey. Your answers will help them get you the best care and services you need, when you need them.

Your care team can help you get the most from your benefits, such as:

 

Convenient rides — to the pharmacy, senior centers, doctors’ appointments and more*

Annual benefit for hearing aids per ear plus a $0 routine hearing exam.

Routine vision exam and an allowance for non-Medicare covered prescription eyewear, including glasses, lenses, frames, or contacts.

Annual benefit to use toward dental services such as X-rays, cleanings, fillings, crowns, extractions, root canals, implants and much more.

A quarterly allowance to buy a wide range of certain over-the-counter (OTC) medicines and health and wellness products.

New in 2022, most plans will include a Healthy foods card allowance. It’s for members to spend each quarter on healthy food at approved stores.
