{VO:} What if a single phone call could put your loved one on a path to better health?

{VO:} What if your loved one had a personal team of experts partnering with them to achieve their best health and wellness?  

{VO:} For Aetna Medicare Dual Special Needs members, like Barbara, these 'what-ifs' can be possible. Thanks to a quick health survey, we learned how we could better support Barbara.

{Female VO:} Hi, I’m Rebecca, Barbara’s care manager. As part of her support team, I’ve gotten to know her well.

{VO:} To begin our partnership, Barbara’s care coordinator gives her a call to learn about her health, living and social needs. April, her daughter and caregiver, joins the call to help answer some of the questions.

{VO:} Then Barbara’s personal care team — which includes a care coordinator, a social worker and me, her care manager — review this information. We work together to create her personalized care plan and share in her journey to better health.

{VO:} Barbara’s care team doesn’t end there. She also has a doctor, nurse, pharmacist, and mental health expert, all joining together to coordinate her care.

{VO:} Speaking with Barbara’s daughter April, we learned that Barbara has diabetes. But she hasn’t been to able to see her doctor for awhile because she doesn’t have transportation.  

{VO:} With that critical information, I reached out to her doctor. I was able to immediately schedule an appointment for a check up — to review her current health status, diabetes medications and to make sure her tests are up to date. I also set up transportation to take Barbara to and from the appointment.

{VO:} The initial phone call was just the start of our relationship. I frequently check in with her to see how she’s doing. This includes making sure her diabetes medicines are filled, and that she’s on track with her testing needs. I also help connect her with resources in the community to access healthy foods, and help her order supplies or medications and have them delivered to her home. As I’ve gotten to know Barbara better, I’ve learned that she’s an ace bridge player. So I’ve connected her with a nearby community center that hosts a weekly bridge game.

{VO:} With her care plan in place, Barbara’s diabetes is under control and she’s feeling a lot better. She’s also making new friends. And for the first time in a long time, April has some peace of mind. Aetna is committed to helping members like Barbara and lightening the load for caregivers like April.
